The Latest Recommendations for Concussion Recognition and Treatment: Insights from the 6th International Consensus Statement

Immediate symptoms can include headache, dizziness, and confusion, but new recommendations suggest a broader symptom assessment to include delayed and subtle presentations. Notably, any suspected concussion should prompt immediate removal from play, with initial assessments incorporating the latest versions of tools like the SCAT6 and Child SCAT6 for standardized evaluation.


The consensus emphasizes a multi-modal approach, suggesting that observation should extend beyond initial signs to catch later-developing symptoms. Importantly, the SCAT6 tool has been refined to include a more comprehensive symptom list and better cognitive and balance testing to improve early diagnosis accuracy.


Rest and Gradual Return to Play


The consensus maintains that while rest is critical in the initial 24-48 hours post-injury, prolonged rest may not be beneficial and can delay recovery. Instead, a more structured, gradual return-to-activity protocol is now recommended.


After the initial rest period, athletes can progressively reintroduce light cognitive and physical activity, as tolerated. The guidelines encourage daily monitoring and adjustments based on symptom persistence, recognizing that each concussion case may require individualized timing for recovery milestones. In addition, new protocols now emphasize the importance of sleep and nutrition as supportive elements in recovery.


Cognitive and Physical Rehabilitation


The 6th consensus introduces a more structured role for cognitive and vestibular-ocular rehabilitation, acknowledging that these therapies can speed up recovery in patients with prolonged symptoms. Referral to specialists in concussion rehabilitation may benefit athletes experiencing challenges in returning to baseline. The consensus suggests integrating light aerobic and balance exercises earlier than before, with modifications as needed for each individual.


Education and Prevention


Education is now highlighted as a crucial element of concussion management. The consensus calls for more robust education for athletes, coaches, and families about the risks, symptoms, and steps for effective concussion management. Preventative strategies, such as proper tackling techniques and protective equipment, are increasingly emphasized, particularly in high-risk sports like rugby and football.
